This form is for the use of employees working ticket/call center positions.
- The end of the day log provides your supervisor/manager with information on how what you may have experienced or had issues with day to day.
- The more you fill out the log the more info we can receive to improve training, fix issues or know what may or may not be working.
- We ask you try to fill out the end of the day log as frequently as possible.
